    Impact of a Web-Based Exercise and Nutritional Education Intervention in Patients Who Are Obese With Hypertension: Randomized Wait-List Controlled Trial

    Background: Internet-based interventions are a promising strategy for promoting healthy lifestyle behaviors. These have a tremendous potential for delivering electronic health interventions in scalable and cost-effective ways. There is strong evidence that the use of these programs can lead to weight loss and can lower patients’ average blood pressure (BP) levels. So far, few studies have investigated the effects of internet-based programs on patients who are obese with hypertension (HTN). Objective: The aim of this study is to investigate the short- and long-term efficacy, in terms of body composition and BP parameters, of a self-administered internet-based intervention involving different modules and learning techniques aimed at promoting lifestyle changes (both physical activity and healthy eating) in patients who are obese with HTN. Methods: A randomized wait-list controlled trial design was used. We recruited 105 adults with HTN who were overweight or obese and randomly assigned them to either a 3-month internet-based intervention group (n=55) or the wait-list control group (n=50). We assessed BMI (primary outcome), body fat mass (BFM), systolic (S)BP and diastolic (D)BP, blood glucose and insulin levels, physical activity levels, and functional capacity for aerobic exercise at Time 0 (preintervention) and Time 1 (postintervention). All the patients in the wait-list control group subsequently received the intervention, and a secondary within-group analysis, which also included these participants, was conducted at Time 2 (12-month follow-up). Results: A 2-way mixed analysis of covariance showed a significant decrease in BMI, BFM, and blood glucose at 3 months in the internet-based intervention group; the effect size for the BMI and BFM parameters was moderate to large, and there was also a borderline significant trend for DBP and insulin. These results were either maintained or improved upon at Time 2 and showed significant changes for BMI (mean difference −0.4, 95% CI −0.1 to −0.6; P=.005), BFM (mean difference −2.4, 95% CI −1.1 to −3.6; P<.001), DBP (mean difference −1.8, 95% CI −0.2 to −3.3; P=.03), and blood glucose (mean difference −2, 95% CI 0 to −4; P=.04). Conclusions: Implementation of our self-administered internet-based intervention, which involved different learning techniques aimed to promote lifestyle changes, resulted in positive short- and long-term health benefits in patients who are obese with HTN

    Micelles of Progesterone for Topical Eye Administration: Interspecies and Intertissues Differences in Ex Vivo Ocular Permeability

    Progesterone (PG) may provide protection to the retina during retinitis pigmentosa, but its topical ocular supply is hampered by PG poor aqueous solubility and low ocular bioavailability. The development of efficient topical ocular forms must face up to two relevant challenges: Protective barriers of the eyes and lack of validated ex vivo tests to predict drug permeability. The aims of this study were: (i) To design micelles using Pluronic F68 and Soluplus copolymers to overcome PG solubility and permeability; and (ii) to compare drug diffusion through the cornea and sclera of three animal species (rabbit, porcine, and bovine) to investigate interspecies differences. Micelles of Pluronic F68 (3–4 nm) and Soluplus (52–59 nm) increased PG solubility by one and two orders of magnitude, respectively and exhibited nearly a 100% encapsulation efficiency. Soluplus systems showed in situ gelling capability in contrast to the low viscosity Pluronic F68 micelles. The formulations successfully passed the hen’s egg-chorioallantoic membrane test (HET-CAM) test. PG penetration through rabbit cornea and sclera was faster than through porcine or bovine cornea, although the differences were also formulation-dependent. Porcine tissues showed intermediate permeability between rabbit and bovine.     Influence of nutrient inputs from a wetland dominated by agriculture on the phytoplankton community in a shallow harbour at the Spanish Mediterranean coast

    [EN] The Safor Wetland (Western Mediterranean) is a protected ecosystem declared Site of Community Importance under the Habitats Directive. Agricultural practices have been part of this ecosystem throughout history, and its hydrology is anthropogenically manipulated to satisfy cultivation needs. Freshwater from the wetland is discharged through surface channels to Gandia Harbour, a shallow water body with high water residence time. This study evaluated the linear eutrophication gradient downstream from the freshwater inflow locations. The role of the main nutrients in determining the phytoplankton community is discussed. The predominance of agricultural practices, 48% of the watershed soil, caused an excess of nitrogen and an imbalance in the nutrient ratios at all the sampling points. Phosphorus concentrations were particularly low, and did not exceed 1.0 ¿M. Chlorophyll-a concentration was of the order of that found in other eutrophic estuarine waters. In general, flagellates dominated over diatoms at all the harbour sampling points and depths. Potentially blooming species of both phytoplankton groups were detected. The correct implementation of the existing agricultural best management practices should continue to reduce nitrogen and phosphorus loading to the estuary.     Perception of university students about doctors and quality of health care provision at clinics: a multi-national study in India, Pakistan, Spain and United States of America

    Background: Patient satisfaction is considered as an indicator of the healthcare quality. Information on patient satisfaction based on medical expertise of the physician, interpersonal skills, physician-patient interaction time, perception and needs of the patient allow policymakers to identify areas for improvement. Primary care services and healthcare structure differ between the countries. The present study was done to determine and analyze the determinants associated with patient satisfaction in India, Pakistan, Spain and USA.Methods: This descriptive study was performed in January to August 2019 among students from Mumbai University, India, Dow University of Health Sciences, Karachi, Pakistan, University CEU Cardenal Herrera, Valencia, Spain, Texas State University, Texas, USA. On the basis of the eligibility criterion (those who gave a written informed consent and were registered students of respective university) 890 (India: 369, Pakistan: 128, Spain: 195, USA: 99) students were selected for the present study.Results: India had almost similar male (49%) to female (51%) ratio of participants. For other 3 countries (PK, ES, US), female participant percentage was nearly 20% or even more as compared to male participants. Overall participant’s satisfaction score about medial expertise of the doctor were highest in India (71%) and were lowest in Spain (43%). Overall satisfaction score about time spent with doctor were highest for India (64%) and were lowest for Spain (41%). Overall satisfaction score about communication with doctor were highest for US (60%) and were lowest for PK (53%). Overall satisfaction score for medical care given by the doctor was lowest in PK (43%) and was highest in US (64%). Overall satisfaction about doctor, highest number of US (83%) and lowest number of PK (32%) participants were satisfied about medical interaction with doctors.Conclusions: These multi-country findings can provide information for health policy making in India, Pakistan, Spain and USA. Although the average satisfaction per country, except Pakistan is more than 60%, the results suggest that there is ample room for improvement

    Resumen: Introducción: El presente artículo forma parte de un proyecto que pretende formar a estudiantes en la observación de la enfermedad utilizando para ello obras de arte. Los objetivos son desarrollar el interés científico de los participantes, promover y desarrollar la afición por el arte en estudiantes con formación básicamente científica. La peste negra, aunque fue una de las mayores catástrofes del mundo pre-modernista, produjo un obligado avance en la medicina. En este contexto histórico, a través de la representación de san Roque se vislumbra el impacto social de la pandemia gracias a la herencia artística del gótico, del renacimiento y del barroco (siglos xiv a xvii). Material y métodos: Se han analizado algunas obras pictóricas sobre san Roque y, aprovechando la visión artística e histórica de su figura, se ha recopilado información de la labor y relación de san Roque con la peste bubónica. Resultados y discusión: A través de contenidos artísticos de la figura san Roque se observa una correlación bastante fidedigna entre la clínica representada en los cuadros de san Roque y los signos descritos en la medicina interna e infecciosa actual. Los signos y síntomas comunes de la peste aparecen fielmente representados en las obras analizadas dado que algunos de los pintores que dedicaron trabajos a su figura convivieron con la enfermedad e incluso la padecieron. Abstract: Introduction: This article forms part of a project intended to teach students about observation through artwork. The objectives were to promote and develop art appreciation in science students, as well to continue developing their scientific interest. Although bubonic plague was one of the greatest catastrophes in the pre-modern world, it was also partly responsible for major advances in medicine. In this historic context, and using the figure of Saint Roch, a Catholic saint and confessor especially invoked against the plague, the social impact of the pandemic is assessed using the artistic heritage of gothic, renaissance and baroque periods (fourteen to seventeenth centuries). Material and methods: Some paintings of Saint Roch have been analysed by looking at the historical and artistic viewpoints in order to gather information about his work and the relationship of the Saint with the bubonic plague. Results and discussion: Through the artistic representations of Saint Roch, and after a bibliographic review, a good correlation can be established between the clinical signs shown in the paintings and those described by in current internal medicine and knowledge of infections. There is ample evidence showing the fidelity of the representations regarding the common signs and symptoms of Black Death, particularly as some of the painters who painted the Saint lived at the time of the epidemics and even suffered the disease themselves.     Topical Ocular Administration of Progesterone Decreases Photoreceptor Cell Death in Retinal Degeneration Slow (rds) Mice

    Retinitis pigmentosa (RP) is an inherited eye disorder which triggers a cascade of retinal disorders leading to photoreceptor cell death and for which there is currently no effective treatment. The purpose of this research was to study whether ocular administration of a solution of progesterone (PG) in &beta;-cyclodextrins (CD) could delay photoreceptor cell death and counteract the gliosis process in an animal model of RP (rds mice). The possible effect of PG reaching the contralateral eye through the circulatory system was also evaluated. Finally, this research discusses and evaluates the diffusion of the drug from possible topical formulations for ocular administration of PG. A group of rds mice received one drop of a solution of PG in CD every 12 h for 10 days to the left eye, while the right eye was left untreated. Another group of rds mice (control) received the drug vehicle (PBS) on the left eye and, again, the right eye was left untreated. Once the treatment was finished on postnatal day 21, the animals were euthanized and histological immunofluorescence studies (TUNEL, GFAP, and DAPI staining) were carried out. Our results showed that the administration of a solution of PG in CD (CD-PG) as drops significantly decreased cell death and inflammation in the retina of the PG-treated eyes of rds mice. No effect was seen in the contralateral eye from PG that may have entered systemic circulation. In conclusion, CD-PG applied topically as drops to the eye decreases photoreceptor cell death in the early stages of RP, delaying vision loss and decreasing gliosis

    Comparison of two sedation protocols for long electroretinography in horses using the Koijman electrode

    Abstract Background In modern times, horses are utilized not only for labour and transportation purposes but also for recreational activities such as competition and pleasure riding. In these various pursuits, the role of vision plays a crucial role. Electroretinography is the most used test to diagnose diseases of the retinal outer segment. There is a wide variety of devices to perform the electroretinography differing one from each other in the corneal electrode and the light stimulation. The Koijman electrode has been tested in dogs but not in horses. The main purpose of this study was to compare electroretinography parameters from horses sedated with detomidine alone or in combination with butorphanol, during a standardized protocol using the Koijman electrode and RETI-port® system. Seven mares were allocated to the detomidine and detomidine plus butorphanol group in a randomised, controlled, crossover study. Friedman and Willcoxon-signed ranked tests were used to compare the electroretinogram parameters. A Student’s t-test was used to compare differences in the number of artefacts to valid values ratio obtained under both sedation protocols. Results Dark adaptation peaked after 16 min under scotopic conditions in both groups. No significant differences in electroretinogram parameters between groups were observed. During the mixed rod and cone response evaluation under scotopic conditions, all mares made a movement of the head resulting in a high number of artefacts. The detomidine plus butorphanol group showed a non-significant tendency to have fewer artefacts and a longer duration of sedation compared to the detomidine group. Conclusions Detomidine alone or combined with butorphanol may be suitable to use Koijman electrode and the RETI-port® to perform a standardized long protocol in horses with some adaptations
